PRESENTATION OVERVIEW

Presentation Title: Incident Investigation through the lens of Human and Organizational Performance (HOP)

Presentation Description: The intent of this session is to give learners an opportunity to understand the nature of incidents through the perspective of HOP. Incidents are not intentional, willful disregard for safety, or unpredictable, but identifiable normal deviations from prescribed work practices. These deviations are the result of failures in knowledge, resources,  competency, feedback loops, and other characteristics of the systems in which the employee operates. Throughout this presentation, learners will be guided to look at the context of the work and challenges that lead up to events, some of which result in a fatal or life-changing event.

LEARNING OBJECTIVES (three to five specific takeaways)

By the end of this presentation, participants will be able to:

  • Learners will be able to take away that human error is normal
  • Learners will be able to explore and understand the concept of organizational drift and how it can result in accidents/incidents/near misses.
  • Learners will consider the issues of context in incident investigations, and the various task demands and organizational capabilities or lack thereof that result in unexpected failures.
  • [Optional learning objective 4]
  • [Optional learning objective 5]

RELEVANCE TO ASSP MEMBERS

This presentation is relevant to ASSP Members within all manner of organizations. The 5 elements of Human and Organizational Performance can help learners understand the work.
